Each device is identified and labeled, indicating the date and the time it was put in use along with the name and title of the person who is authorized, and the purpose of the lockout or tagout (repairs maintenance, repairs and so on.). It must be clearly stated that these devices are not to be removed until the LOTO procedure is complete.
